Changes to Name, Ownership and Entity
Name change, change in ownership, or entity change requires an ERM-14(PDF) and new application be submitted. In order to make changes, the ERM-14 must be completed in full.
Waiver of Subrogation
If an insured is requiring a Waiver of Subrogation(PDF) be added to their policy, please have them complete the appropriate waiver form and return to our office via email at: endorsements@atlas.us.com.
Please contact our office to check on the availability of waivers per carrier to ensure that the waiver can be endorsed onto the policy.
