Please note, bind requirements include:
1. Formal order to bind (please attach copy of quote if available)
2. Confirmation of payment plan (MR or installments and % deposit)
3. Signed ACORD 130 Application by insured and producer (please be sure
the payroll and EE count per class code, per location is complete)
4. Copy of deposit check
5. E-mail confirmation with bind order stating that insured will enroll in
Athens MPN (MedEx) *For Companion and Ullico binds only
