Changes to Name, Ownership and Entity
- Name change, change in ownership, or entity change requires an ERM Form (PDF) and new application be submitted. In order to make changes, the application must be completed in full.
Waiver of Subrogation
- If an insured is requiring a Waiver of Subrogation (PDF) be added to their policy, please have them complete the appropriate waiver form and return to our office via email at: firstname.lastname@example.org.
- Please contact our office to check on the availability of waivers per carrier to ensure that the waiver can be endorsed onto the policy.
